Meningioma / hemangiopericytoma presenting as proptosis


This 60yr old male patient presented with gradual, but progressively increasing proptosis, over the past 10 years. There was no history of diplopia. He had developed pain in right orbit, for which he took medical help.






Axial T2 WI showed increased distance between the interzygomatic plane and the anterior surface of the cornea, consistent with right sided proptosis.



Lesion appears isoto hypointense, in the pre contrast images.


Post contrast MP-RAGE 3D T1 sequence shows abnormally enhancing extra-axial lesion in right inferior frontal and anterior temporal region, which showed features similar to that of a meningioma. However CT showed destruction of the greater wing of sphenoid and adjacent frontal bone, with a permeative destructive pattern.



A large fronto-temporo-parietal dural tail is noted in the post contrast images.



Lesion is showing intensely enhancing soft tissue component with both intracranial and intraorbital soft tissue component. Mass effect on right lateral rectus muscle was noted. Right eye showed proptosis. Lesion characteristics favour a hemangiopericytoma or a meningioma (invasive).
